Under the Data Protection Act (1998), patients have a right of access
to their medical records, except where the doctor judges that the release of
certain information would be damaging to the patient’s health, or would
compromise the confidentiality of another third party.
Requests to access medical records must be made in writing to:
The Practice Manager
The Castle Medical Group
The Health Centre
Railway View Road
Clitheroe
Lancashire BB7 2JG
The Practice Manager will need to discuss the request with your GP, and then
will contact you. Please include a day-time telephone number in your letter if
possible, so that a suitable appointment may be made.
You may only view your medical records on the premises (you may not take your
medical records away from the premises).
Whilst you may be allowed to view your records, you will not be allowed to
alter them in any way.
We can provide photocopies of letters, test results and other records, on
request – there will be a charge of 10p per copy.
