If you wish to make a complaint regarding any aspect of the practice or the care that you have received, this should be put in writing to the Practice Manager using our Complaint Form.

If you wish to make a complaint on behalf of another person, you will need to also complete our Complaint – Third Party Consent Form.
Copes of both the complaints form and leaflet are available in hard copy from reception.
